INSURANCE

Our office accepts any private insurance that allows you to choose your own dentist. If your insurance has an “in network” or “out of network” phrase it simply means you can choose your doctor, but if they are out of the network you may pay a little more.


SCHEDULING NEW PATIENTS

All new patients are welcome. It is appreciated if patients arrive 15 minutes early for their first visit to fill out forms. We are more than happy to check eligibility and coverage before you arrive. Please understand we will need the name of the insured, employer, birthday and social security number.

New patients that are not emergencies are scheduled for two appointments. The first visit is a full exam and x-rays. The second is a consultation and cleaning.

Deductibles are usually collected once a year for major work. However, some insurances may have variables in their policies.


COPAYMENTS

Copayments are the portion your insurance does not cover. Insurance companies may cover 100% – 50% or less. Whatever portion is left is the patients responsibly. The payments are due at the time of the visit.

CANCELLATION POLICY

If it becomes necessary to change an appointment please call 48 hours in advance. This way, someone else may have the opportunity to use that time. There may be a charge for cancellations without 48 hour notice.

FINANCIAL POLICY

All patient portions are due at the time of the visit. We accept cash, check, MasterCard and Visa.
